Our Case Managers are in your corner

RefuahHealth’s Case Management team helps connect Members to federal, state and local benefits and services including affordable insurance, social services and community resources. We recognize that our Members’ needs often extend beyond healthcare. Obstacles such as a finances, basic food and shelter needs, emergency situations, and more can affect quality of life and may interrupt consistent access to healthcare.

Community Resources

Even when families do not qualify for state or federally funded programs, we understand that they may still have unmet needs. There are many community resources within Rockland County to assist families and individuals who are struggling. RefuahHealth can help connect Members with various programs including:

  • Smoking Cessation
  • Food Pantries
  • Clothing
  • Substance Abuse
  • Immigration Services
  • Domestic Violence
  • Education Assistance
  • And more…

Social Services

Our Case Managers also assist Members in applying for social service programs and benefits that provide financial and basic needs assistance to families and individual.  Programs include:

  • Social Security Benefits
  • Social Security Card Applications
  • Temporary Assistance
  • SNAP/Food Stamps
