Revitalizes Healthcare for New Yorkers

Refuah Health Center is the lead agency of the Refuah Community Health Collaborative (RefuahCHC), the only Performing Provider System (PPS) led by a Federally-Qualified Health Center (FQHC). RefuahCHC partner organizations provide access to high-quality care and services across Rockland & Orange Counties. Though one of the smallest, RefuahCHC is the highest performing PPS in New York State.  RefuahCHC is population-focused and committed to serving the individual and improving the community. Hands-on leadership and PPS partner’s dedication and effort ensures continued success.

*The Delivery System Reform Incentive Payment program, better known as DSRIP, will provide $6.42 billion to support the transformation of New York State’s health care delivery system.  The program aims to improve care and outcomes and reduce costs for Medicaid beneficiaries. The State’s primary objective through DSRIP is to reduce avoidable hospitalizations by 25% statewide over five years. DSRIP is currently in year 5.

To achieve these goals, the program supported the creation of 25 regional, integrated Performing Provider Systems (PPS).  The PPSs are tasked with developing and implementing innovative strategies to improve care and reduce costs for Medicaid populations.
