RefuahHealth Members have all of our resources at their disposal, this includes our vast array of non-clinical support. From transportation to insurance enrollment services, the Patient Services Department helps connect Members with the resources they need to thrive.
Our team works with Members to identify needs and find available social services. Our assisters ensure that Members with complex medical needs receive the highest quality of care by scheduling outside consultations and referrals, and locating medical equipment or medications. Patient Advocate Services
RefuahHealth is in your corner. Our Patient Advocate is an anonymous ear, helping Members effectively communicate with doctors, solve problems, and keeping patients safe and informed. Our advocates gives Members a voice on health issues that matter most to them.
Patient Advocates help with:
- Patient rights
- Privacy issues
- Informed consent
- Patient representation
- Support and education of patients and their caregivers

What to Bring to Your Appointment
After you make your appointment, please bring these forms to your visit. If you are having a hard time with your forms, the Patient Services Department can help you fill them out.
- Patient Registration Form (Online Form or PDF: English • Español)
- Insurance card and copay
- If you do not have insurance but need help paying, bring proof of your family’s income to your appointment.
- If you need a lower cost choice, please bring proof of how much money your family makes. Our sliding scale fee is based on household size and how much money the household makes each year.
- For patients who do have insurance, or may be eligible for insurance, here is a list of Insurance Plans Refuah Health Center accepts: Download the complete list of accepted plans
- If you do not have insurance but need help paying, bring proof of your family’s income to your appointment.
- Records from your last doctor’s office
- Shot cards or vaccine records
