Refuah Community Health Collaborative
Advancing Care for New Yorkers Through Proven Leadership
During New York State’s Delivery System Reform Incentive Payment (DSRIP) program, Refuah Health Center served as the lead agency of the Refuah Community Health Collaborative (RefuahCHC)—the only Performing Provider System (PPS) in New York State led by a Federally Qualified Health Center (FQHC).
Through RefuahCHC, partner organizations came together to deliver high-quality, coordinated care across the region. RefuahCHC consistently ranked among the highest-performing PPSs in New York, demonstrating that strong outcomes can be delivered by vision, collaboration, and hands-on leadership.
Throughout the DSRIP program, RefuahCHC remained population-focused and deeply committed to improving both individual health outcomes and overall community well-being. The collaborative’s success was rooted in active leadership and the dedication of its partners, advancing primary care transformation, behavioral health integration, and strengthened safety-net services across the region. The impact of this work continues to inform and shape Refuah’s ongoing approach to community-based care.
